Guidelines of Documents to Accompany a Recognition of Qualifications (ROQ) Application Form for Clinical Measurement Physiology Qualifications Received Outside This Jurisdiction

 

To be provided by the applicant in English

  1. Copies of undergraduate and postgraduate certificates of qualifications or awards received to be accompanied they must have a comparability statement from QQI /NARIC supplied with them.
  2. Additional qualifications/certificates relevant to the profession to be accompanied by a QQI or NARIC comparability statement.
  3. Certified copies of official transcripts for qualification (s) awarded.
  4. Certified copies of official certificates / transcripts for all other relevant qualifications.
  5. Certified description of course (s) content – course Syllabus/handbook.
  6. Name and contact details for the regulatory body where applicable in the country where qualification was awarded.
  7. Registration Certificate to be provided by the authority/regulatory body in the country where qualification was obtained with registration number (if applicable).
  8. And/or evidence of membership of a professional body appropriate to Clinical Measurement Physiology with listed membership number (if applicable).
  9. Certified description of course (s) content – course Syllabus/handbook.
  10. Certified documentary evidence of eligibility to practice.
  11. Certified copy of proof of birth certificate.
  12. A certified and signed copy of photographic identification e.g. Passport/driving licence.
  13. Certified copy of evidence of change of name (if applicable/different to name on qualification certificates).
  14. Name and contact details for the regulatory body where applicable in the country where qualification was awarded.
  15. Names and contact details of persons named on certification documents, translators, educational institution contact and solicitor/notoriety involved.
  16. Certified translation of all documents into English and electronic copies of all documentation in original language to be included.
  17. Certified copies of Erasmus application, Erasmus Learning Agreement, Erasmus confirmation of attendance, Erasmus participant’s survey and complete transcripts.
  18. Consent to background checks.
  19. Confirmation from current employers of current position in their employment

*Please note that the college may email section 3 of the application form and the supporting documents in the original language to the HSE however, the applicant is required to supply the certified translation of these documents with their full application

To be provided by the educational institution

  1. Section 3 of the application form must have each page signed and stamped by the educational institution.
  2. Certification of Qualification awarded.
  3. Transcripts for each year of the course studied. Including course code.
  4. Course syllabus/handbook including subject content plus number of hours studied.
  5. Certified signed Internship certificate.
  6. Certified signed relevant qualifications:
    • Final Award Certificate
    • Academic Record
    • Intern Certificate
    • Official transcripts of course to include, required hours of study for each subject taken plus practical placements.
  1. Certified copies of Erasmus transcripts for all subjects taken during the Erasmus.

To be provided by previous employer

  1. Completed and certified section 6 of the application form.
  2. Confirmation of employment is required from your current employer or your most recent substantial employer.
